The Junior engineers on the Death Star knew about thermal exhaust port, but the Empire didn't have an Architectural Review Board and at the meeting where it was brought up to the Senior Architect, the concern was met with 25 minutes of gish-gallop and the various Project Managers didn't know who to believe. The Thermal Exhaust Port Manufacture Union wants people to know that blame is misplaced and this is shoddy root cause analysis.
